Information you send us
When you use a request, interest-list, or partner form we collect the details you enter, such as your name, email, optional phone number, state or ZIP code, the service you are interested in, and your message. We ask you not to send medical records or sensitive health details through these forms.
How we use it
To respond to your request, route it to the right service or location, and keep a record of our communication. Form submissions are emailed to our team and stored in our website administration area. We do not sell your information and we do not add you to marketing emails unless you separately opt in.
The free Wellness Snapshot
Your snapshot answers are processed in your own browser. They are not sent to our server, stored, emailed, or shared with analytics or advertising tools.
Booking and payments
When online booking is available, appointments, intake forms and payments are handled by our booking and payment providers under their own security and privacy terms. Card details go directly to the payment processor; we never see your full card number.
Client services
If you become a client, analysis results, measurements, and intake information are kept in a protected system separate from marketing records and are shared with others (for example a trainer) only with your authorization.
Home visits
If you choose a home visit, we collect your address and access notes only to schedule and travel to your visit.
Service providers we use
Some services run on third-party platforms: the Energy 4 Life analysis (which may use a short voice recording), Fit3D body-scan reports, and VALD strength-test reports. We tell you what each one collects and ask for your consent before using it.
Cookies and analytics
The website uses cookies needed for it to work. Our hosting provider may collect basic, aggregated traffic statistics. We do not use advertising pixels or session recording on forms or the score.
Retention and your choices
We keep inquiry records only as long as needed for the purposes above and our legal obligations. You may ask to access, correct, or delete your information by emailing health@biovitality.co.
Changes
We will update this page if our practices change.
